Responding to the report on 14 March, Children’s Commissioner for Wales Rocio Cifuentes said:
“Today’s report is clear that Kaylea was let down and that her indescribable suffering was avoidable. There was no effective oversight of Kaylea’s care and support needs, significant deficiencies in co-ordination between agencies, and a disjointed approach across health services to responding to missed appointments. I’ll be urgently discussing these points and other issues raised in the report with the local authority and the health board. I’ll also be pressing the Welsh Government on how they will make sure that learning from this review will improve practice across Wales.”
